Fallen transverse arch (splayfoot)

Splayfoot is a common condition that can cause pain and a burning sensation under the forefoot, especially when weight is placed on it. The condition often develops gradually and can affect both walking and everyday activities. Here you will get a clear overview of what splayfoot means, common causes and symptoms, when you should seek care and how the right support and shoes can help relieve the pressure.

What is splayfoot?

Splayfoot, or fallen transverse arch, means that the transverse arch at the front of the foot sinks down and widens. As the arch loses its elasticity, pressure increases on the heads of the metatarsal bones, which can lead to pain and tenderness in the ball of the foot. The condition often develops gradually over time and is associated with weakening of the muscles, tendons and ligaments that normally help to hold the forefoot up.

Common causes & risk factors

Common causes include prolonged stress on hard surfaces, weakness in the small muscles of the foot, and the use of narrow or high-heeled shoes that increase pressure on the forefoot. Age-related changes, obesity, and previous injuries to the foot can also contribute. People who stand or walk a lot at work may be at increased risk of developing a splayfoot.

Symptoms

Symptoms may include pain or a burning sensation under the forefoot, especially with weight bearing. Many people experience tenderness in the ball of the foot, calluses under the metatarsal bones, or a feeling of “walking on cushions.” The discomfort is often worsened with longer walks or when wearing shoes with thin soles and limited cushioning.

When should you seek medical attention?

Seek medical attention if the pain is persistent, if your gait is clearly affected, or if the symptoms do not improve despite relieving the pain and changing shoes.

In case of severe pain, swelling or suspected fracture, a medical assessment should be performed.

Recommended protection & support

Insoles with a forefoot pad (cushion behind the ball of the foot) can help lift and relieve the transverse arch of the foot and reduce pressure on sensitive areas.

Shoes with a spacious toe box and good cushioning can reduce the strain on the forefoot. Training the small muscles of the foot can also contribute to improved stability and function over time.
